Request Operating Health Facilities

The following are the steps to download master facility lists

1. Complete below form. Make sure you provide all necessary details for your request
2. Attach letter from your organization
3. Your request will be reviewed by the Ministry
4. If your request approved , the master facility list will be sent to your email
5. If your request is rejected , you will be notified through your email
